The HITZ 104.9 Discount Fireworks Contest starts on 6/24/2019 and ends on  7/2/2019.  To enter, sign up online for your chance to win.  Winner will receive a $250 gift certificate for use at either Visalia location of Discount Fireworks Superstore.  Prize will be awarded on 7/2/2019 and must be picked up at HITZ 104.9  office located at 700 East Mineral King Avenue, Visalia CA 93292 by 1pm on July 3, 2019.  Prize must be used by July 4th, 2019.  This prize has a retail value of $250, and one prize will be awarded.  Prize is not refundable or transferable, and must be used by prize winner or forfeited.  Winner must be 18 years of age or older, and will be required to execute the release of liability below in order to take possession of this prize.  Failure to complete the release of liability will result in forfeiture of prize.   There are no substitutions or exchanges for cash or credit for any prize at any time. Resale for any reason is strictly prohibited. I acknowledge in accepting this certificate good for fireworks at a Discount Fireworks store that there are in fireworks the potential for death, injury, and property loss.  In accepting this certificate I hereby exclusively assume all risks related to the possession and use of the fireworks I will be acquiring.

I acknowledge that Momentum Broadcasting, its companies, radio station employees, and advertisers are not responsible for any action by me, my associates, or any recipient of the Discount Fireworks acquired with the use of the Discount Fireworks gift card I am receiving.

I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:  (A) Waive, release, and discharge from any and all liability for my death, disability, personal injury, damage of property owned by myself or others, or actions of any other kind which may hereafter accrue to me or my associates  (B) Indemnify and hold harmless all entities or persons mentioned in this statement from any and all liabilities or claims made by other individuals or entities as a result of my actions or my associates actions in the use of the fireworks acquired via the use of this gift card.

I understand that this Discount Fireworks Certificate will allow me to acquire fireworks that are to be used in a safe manner, and that I and all my associates involved in their possession and use will abide by all rules set forth by Federal, State, and Local authorities regarding their use.

I hereby certify that I have read this document accepting 100% of the liability associated with the items I will acquire as the result of execution of this gift card.  I further certify that I am 21 years of age or older, and that I understand and agree to all its stipulations.
